Ravinia PTO Reimbursement Form

All purchases related to the Ravinia PTO must be approved by the Committee VP or one of the Co-Presidents.

Reimbursement requests exceeding $250.00 MUST BE PRE-APPROVED by the Co-Presidents and Treasurer.

Reimbursement requests less than $5.00 will not be processed.

In order to receive a reimbursement for approved expenses related to the Ravinia PTO, the below form must be submitted within 14 days of purchase and/or conclusion of the related PTO event.
